By using Cortex Health’s (Australian Echolight distributer) services you understand that Cortex Health is the
Australian supplier of Echolight, a TGA approved registered medical device for the measurement of bone
mineral density and subsequently the diagnosis and detection of osteoporosis. Cortex Health is NOT a
registered provider of medical services.

A Radiofrequency echographic multi spectrometry (REMS) scan via Echolight is a non-invasive procedure
and performed by a trained technician. Cortex Health is obligated to and will send the report/s to your health care practitioner on your referral.

1) You understand that this scan is designed to be an ancillary health aid and Cortex Health should not be
construed as a substitute for medical examination, diagnosis, or treatment and that you should see a
qualified medical specialist for the interpretation of the REMS report. You understand that Cortex Health
technicians are not qualified to diagnose, prescribe, or treat any physical or emotional conditions and
that nothing said during this scan should be construed as such.

2) By using these services you acknowledge your consent to the performance of the proposed scan by my technician at
Cortex Health. You are able to withdraw your consent at any time.

3) You consent to Cortex Health using your deidentified data for research and reporting purposes. You understand that you can withdraw your consent at any time. – If you would like to opt out this can be done on the Patient Form after making a booking.
